Semaglutide is a synthetic peptide analogue of glucagon-like peptide-1, a gut hormone released by intestinal L cells after food intake. The natural hormone acts on pancreatic and central receptors but is degraded within minutes by dipeptidyl peptidase-4 and other peptidases. Semaglutide belongs to the class of long-acting GLP-1 receptor agonists, a group distinguished by structural changes that slow breakdown and extend circulation time. Its development followed earlier short-acting analogues and reflects a general strategy in peptide drug design: preserve receptor activity while blocking proteolytic clearance.
Three structural changes define the molecule. At position 8 an alpha-aminoisobutyric acid residue replaces alanine, which blocks dipeptidyl peptidase-4 cleavage. In this report, we present a case of rhinoentomophthoromycosis in a middle-aged, immunocompetent male, primarily affecting the nasopharynx. Initially, the diagnosis posed a dilemma, but it was ultimately confirmed through meticulous examination of microscopy and histopathology of the aspirate. The patient's response to antifungal therapy was favorable. We hereby provide a comprehensive account of this case and discuss the efficacy of Posaconazole in the treatment of rhinoentomophthoromycosis in a 55-year-old male patient with diabetes.

Americium is used in the most common type of household smoke detector, which uses 241Am in the form of americium dioxide as its source of ionizing radiation. This isotope is preferred over 226Ra because it emits 5 times more alpha particles and relatively little harmful gamma radiation. The amount of americium in a typical new smoke detector is 1 microcurie (37 kBq) or 0.29 microgram. This amount declines slowly as the americium decays into neptunium-237, a different transuranic element with a much longer half-life (about 2.14 million years). With its half-life of 432.2 years, the americium in a smoke detector includes about 3% neptunium after 19 years, and about 5% after 32 years. The radiation passes through an ionization chamber, an air-filled space between two electrodes, and permits a small, constant current between the electrodes. Any smoke that enters the chamber absorbs the alpha particles, which reduces the ionization and affects this current, triggering the alarm. Compared to the alternative optical smoke detector, the ionization smoke detector is cheaper and can detect particles which are too small to produce significant light scattering; however, it is more prone to false alarms.
=== Bone fractures === GlaxoSmithKline reported a greater incidence of fractures of the upper arms, hands and feet in female diabetics given rosiglitazone compared with those given metformin or glyburide. The information was based on data from the ADOPT trial. The same increase has been found with pioglitazone (Actos), another thiazolidinedione. A meta-analysis of 10 RCTs, involving 13,715 patients and including both rosiglitazone- and pioglitazone-treated patients, showed an overall 45% increased risk of fracture with thiazolidone use compared with placebo or active comparator. It doubled the risk of fractures among women with type 2 diabetes, without a significant increase in risk of fractures among men with type 2 diabetes.

==== Limitations ==== Because relatively few cells are counted in the manual differential, the variability is higher than in automated techniques, especially when cells are present in low amounts. For example, in a sample containing 5 percent monocytes, the manual differential results could be between 1 and 10 percent due to sampling variation. Additionally, cell identification is subjective and the accuracy depends on the skills of the person reading the slide. Poor blood smear preparation can cause an uneven distribution of white blood cells, resulting in inaccurate counting, and improper staining can impede cell identification. Overall, manual differential counts exhibit coefficients of variation (CVs) ranging from 5 to 10 percent, while automated differential counts of normal neutrophils and lymphocytes have CVs of about 3 percent. In leukemias and other hematologic malignancies, the lineage and genetic characteristics of white blood cells have important implications for treatment and prognosis, and the microscopic appearance of the cells is often insufficient for accurate classification. In these cases, other techniques such as immunophenotyping by flow cytometry or special staining can be used to definitively identify the cells.
== Side effects == Side effects of fentanyl analogs are similar to those of fentanyl itself, which include itching, nausea and potentially serious respiratory depression, which can be life-threatening. Fentanyl analogs have killed hundreds of people throughout Europe and the former Soviet republics since the most recent resurgence in use began in Estonia in the early 2000s, and novel derivatives continue to appear. A new wave of fentanyl analogues and associated deaths began in around 2014 in the US, and have continued to grow in prevalence; especially since 2016 these drugs have been responsible for hundreds of overdose deaths every week.
=== Glucocorticoid activity === Progesterone is a partial agonist of the glucocorticoid receptor (GR). It has about 35% of the affinity of dexamethasone, a corticosteroid, for the human GR, and about 3 to 11% of the affinity of dexamethasone for the rat GR. However, progesterone appears to show weak or no glucocorticoid activity and no antiglucocorticoid activity in vitro and in animals. Nonetheless, progesterone has been found to upregulate the thrombin receptor in vascular smooth muscle cells in vitro, a glucocorticoid effect, and this could have clinical relevance in relation to risk of blood clots.
The parathyroid glands, of which there are 4–6, are found on the back of the thyroid glands, and secrete parathyroid hormone. This causes an increase in blood calcium levels by targeting bone, the intestine, and the kidneys. The parathyroid hormone is the antagonist of calcitonin. Parathyroid hormone release is triggered by falling blood calcium levels and is inhibited by rising blood calcium levels.

Quevedo Martín, and the Director of the retirement home, Raúl Acosta to come to the vault and withdraw 1,000 dollars. After being informed of the situation, Grand Master Urquía Carreño said that the elevator in his office was broken, and that Urquía Carreño would bring the money to the offices of the Supreme Council. Grand Commander Viñas Alonso refused, citing the rules. Urquía Carreño said that he understood, and would call later to tell Viñas Alonso what time they could come by the office.
=== Moisture balance === Establishing a moisture balance beneficial to the wound bed is another prerequisite of care. The natural response to injury is inflammation typified by the local expression of histamine and bradykinin and leading to vasodilation of the vessels that are in relative close proximity to the site of injury. As serum based fluid moves out of the vessels into the interstitial spaces the resultant soft tissue oedema manifests on the wound surface as exudate. In the chronic wound this exudate contains a surfeit of proteolytic enzymes and other components not seen in acute wounds and these compounds have a corrosive effect on the wound bed and surrounding peri-wound skin. The application of dressings, topical negative pressure, compression garments and leg elevation/exercise have been identified as methods for management of wound exudate.
